Blair is likely to battle it out with Gloucester's full-back Iain Balshaw for a starting berth in the team. Balshaw, who is joining Biarritz, is named in a squad alongside former England colleague Martin Corry and London Wasps duo Josh Lewsey and Serge Betsen.
As well as Tito and Blair there will be two other New Zealanders in the squad in the form of former All Black stars Jerry Collins and Justin Marshall, as well as Australian Rocky Elsom.
"To have the opportunity to pull on the Barbarians jersey and return to Twickenham Stadium so soon after the EDF Energy Cup final was one I couldn't turn down," said Tito, the Blues captain.
"It's a huge honour to play for the Barbarians and one I'm really looking forward to."
Cardiff Blues director of rugby Dai Young has been made the Barbarians coach for the match.
